Award for Exceptional University Service
Award Description:
This award recognizes and rewards exceptional University service by non-academic staff members who devote time and energy to support and serve the campus community and mission. As the University's success is founded upon the contributions of the people who work at UCSF, service that goes above and beyond the scope of an individual’s job and demonstrates outstanding University citizenship is a critical achievement to be recognized.
Eligibility Requirements for Nomination:
Nominees must meet the following eligibility requirements:

Have been a career UCSF employee for at least five years as of December 31, 2011, and have a minimum 50% staff appointment at the time of nomination (questions regarding employment eligibility can be answered by your Human Resources representative prior to submitting a nomination).

Have been nominated by their supervisor and endorsed by their department head.

Have not previously received the award.

Have exhibited and sustained outstanding service to the University as delineated below by the "Selection Criteria.”
Selection Criteria:
Nominees will be evaluated per the following criteria:

Effort – How much effort beyond the scope of the nominee’s job, research, or training does the nominee give to exceptionally serve the University?

Benefit – How has the University and its priorities: patients, discovery, education, people, and business substantially benefitted from this nominee's service (it is assumed that if other staff members, patients, students or the community benefit, the University also benefits)?

Participation – How much does the nominee exceed expectations in actively participating, interacting, and collaborating with the campus community to provide exceptional service to the University?

Lasting Impact – How has the nominee’s service beneficially impacted the University over an extended period of time?

Base of Operation – How does the nominee’s exceptional service relate to the expectations of their position (some employees in higher level positions/ jobs may have a greater capacity to demonstrate exceptional service more visibly than those in lower level positions)?
Required Nomination Materials:
Please submit a nomination packet either electronically or in hard copy containing the following documents:

Completed nomination form (see below)

Supervisor's objective statements about the nominee's service contributions related to the above “Selection Criteria” (specific examples per each criterion recommended). Please note: three pages maximum, submitted as either a formal letter or Supervisor Statements template (see below), will be accepted.

Two supporting letters from peers who know the nominee's contributions and can give examples of their exceptional service related to the above criteria. Please note: only two supporting letters, limited to two pages, will be accepted.
